Job Summary

We are seeking a skilled Case Manager to join our team at Delta-T Group Inc. in Trenton, NJ. As a Case Manager, you will be responsible for providing case management and assessment services to consumers in an on-site setting. Your primary goal will be to ensure coordinated service delivery and successful client linkage with other programs and facilities.

Key Responsibilities
  • Facilitate small education groups, problem-solving groups, skill-building groups, and larger socialization and therapeutic recreational-related groups and activities.
  • Maintain a caseload of case management and provide outreach for consumers who have missed their appointments.
  • Attend multi-disciplinary team meetings to provide input and obtain feedback about consumer cases.
  • Develop community contacts to secure community referrals and ensure consumers' needs are met.
  • Utilize a person-centered approach to aid consumers in meeting their prioritized needs.
  • Attend client trainings as appropriate to the position or for requirements for the agency to maintain positive, open, and respectful communications with consumers, families, and other involved persons.
Requirements
  • BA/BS in Human Services or a related field and 2 years of work experience.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
