Job SummaryThe Case Manager will be responsible for delivering case management services to clients and families, ensuring they receive the support and resources they need to achieve their goals. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.
Key Responsibilities- Design and implement individual program plans for clients, taking into account their unique needs and circumstances.
- Assess client needs and investigate their eligibility for services, ensuring they receive the most appropriate support.
- Counsel and aid individuals and families requiring agency assistance, providing guidance and support to help them achieve their goals.
- Maintain accurate and up-to-date client records, completing required documentation according to agency and accreditation standards.
- Coordinate client activities and regular meetings, ensuring seamless communication and collaboration.
- Assist clients in accessing other services, such as job training, counseling, housing, and medical services.
- Network with other providers to generate community resources for participants, ensuring they receive the support they need to succeed.
- May provide supportive counseling in individual and group settings, helping clients to overcome challenges and achieve their goals.
- May transport consumers to appointments, ensuring their needs are met and they receive the support they require.
- High School / GED + 5 years of human services experience; OR relevant professional credential such as CPRP, CRC, CADC + 2 years related exp.; OR a combination of one or more years of college + related work or life experience together equaling four years; OR Bachelor's degree in human services field, rehabilitation, nursing, or a related discipline.
- Skills to include case management, database entry, and case file management experience.
- Bilingual (Spanish) preferred.
