Job Summary

Delta-T Group is seeking a highly skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing case management services to consumers to help them gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community.

Key Responsibilities
  • Facilitate small education groups, problem-solving groups, skill-building groups, larger socialization and therapeutic recreational-related groups and activities.
  • Maintain a caseload of case management.
  • Responsible for documenting case management progress notes in a timely fashion.
  • Complete initial applications of referrals on a scheduled basis.
  • Plan, structure, and facilitate and maintain focus in groups to support skill acquisition.
  • Provide outreach for consumers that have missed their appointments.
  • Attend multi-disciplinary team meetings to provide input and obtain feedback about consumer cases.
  • Develop community contacts to secure community referrals to ensure consumers needs are met.
  • Utilize person-centered approach to aid consumers to meet their prioritized needs.
  • Attend client trainings as appropriate to the position or for requirements for the agency to maintain positive open and respectful communications with consumers, families, and other involved persons.
  • May require group facilitating, assessment, or log reporting of consumers.
Requirements
  • BA/BS in Human Services or related field and 2 year work experience.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and analytical skills.
  • Ability to maintain confidentiality and handle sensitive information.
