St. Joseph Center, a 501(c)(3) nonprofit community organization, has been meeting the needs of low-income and homeless individuals and families since 1976. Our comprehensive case management and integrated social service programs assist people without regard for religious affiliation or lack thereof.
We serve approximately 6,000 individuals annually and enjoy broad-based community support as well as a sponsored relationship with our founders, the Sisters of St. Joseph of Carondelet.
Job SummaryThis full-time position is responsible for providing case management services to homeless families experiencing domestic violence. The Service Coordinator will work closely with our team to deliver quality services and adhere to our mission.
Key Responsibilities- Evaluate families for permanent housing in conjunction with the Program Manager and LAHSA's DV Unit case management staff.
- Perform advocacy on behalf of the participant, conduct intakes and exits with families entering and exiting the program.
- Practice Trauma-Informed Care, Housing First, Harm Reduction, and Low Barrier techniques when working with participants.
- Manage a caseload of up to 20 families, establishing a case management and safety plan for each family and developing housing retention plans to address short-term and long-term housing goals.
- Work closely with the DV Regional Coordinator and the LAHSA DV Unit.
- Identify internal and external resources to assist with multiple stressors related to homelessness, domestic violence, substance abuse issues, etc.
- Participate in case conferences, case consultations, and other client-related meetings.
- Provide referrals to needed services, including health, mental health, education, employment, and vocational training.
- Offer housing stabilization services, which may include topics like money management, life skills, accessing transportation, understanding lease agreements, self-advocacy, and other topics based on client needs.
- Avoid conflicts and tensions by adopting a 'whatever-it-takes' approach in assisting clients to retain housing and independent living.
- Conduct quarterly client file reviews to ensure the program meets compliance requirements.
- Provide interagency and intra-agency consultation, communication, coordination, and referrals.
- Cultivate relationships with landlords and other housing providers to build a portfolio of housing options for participants.
- Assist families with identifying appropriate housing and leasing agreements, negotiating rental agreements with landlords as needed.
- Enter data in required databases, compile reports tracking client status and support services provided.
- Provide ongoing case management to families following placement.
- Bachelor's degree from an accredited college or university in social work, human services, psychology, or a related field.
- Demonstrated knowledge of trauma-informed crisis intervention, violence dynamics, domestic violence, intimate partner violence, human trafficking, stalking, child abuse/neglect, stalking, and elder abuse intervention.
- Experience in Crisis Counseling and ability to establish effective working relationships with internal and external partnering agencies.
- One to two years of experience providing case management services/advocacy with homeless and low-income families and/or families experiencing domestic violence.
This full-time position offers a competitive salary range of $45,000 - $55,000 per year, paid bi-weekly. The estimated hourly rate is $21.67 - $26.44 per hour.
Additional benefits include paid time off, holidays, and sick leave. Health insurance, dental insurance, and vision insurance are also offered.
