Job Overview:

The Client Support and Advocacy Professional will play a vital role in ensuring the well-being and independence of vulnerable communities. This position involves working with a multidisciplinary team to provide comprehensive services, including community education, collaborative work with Adult Protective Services, and development of individualized care plans. The ideal candidate will possess excellent communication skills, a strong understanding of cultural sensitivity, and the ability to work effectively in a fast-paced environment.

Duties and Responsibilities:

  • Conduct community-based educational programs to promote awareness and engagement among diverse groups.
  • Collaborate with Adult Protective Services to identify and mitigate risks faced by clients.
  • Assess client needs and develop personalized care plans to ensure seamless service delivery.
  • Document client interactions and progress in a thorough and accurate manner.
  • Explore options for short- and long-term care and support services with clients and their families.
  • Maintain a network of support for clients through regular case monitoring and outreach.
  • Foster relationships with public and private service organizations to enhance community connections.
  • Manage a caseload of up to 40 clients while prioritizing timely and effective service delivery.
  • Collaborate with mental health professionals to provide crisis intervention and support.
  • Work flexible hours, including evenings and weekends, as needed.
  • Participate in regular team meetings and professional development opportunities.
  • Complete all required paperwork, data collection, and billing tasks in a timely and accurate manner.
  • Advocate for clients and their families to address social and economic issues affecting their lives.
  • Travel throughout LA County for meetings, client visits, and other essential activities.
  • Perform other duties as assigned by the Division Manager and Program Coordinator.

What We Offer:

A competitive salary range of $60,000 - $80,000 per year, commensurate with experience, plus benefits and opportunities for professional growth and development.
