Position Overview

ROLE OBJECTIVE

The Director of Rehabilitation Services will oversee a team of Assistant Directors and rehabilitation professionals, ensuring effective coordination and delivery of care within the Certified Home Health Agency (CHHA). This role is pivotal in ensuring the provision of high-quality rehabilitation services, including physical therapy, occupational therapy, and speech-language pathology. The Director will also focus on mentorship and development of the care team to achieve clinical and professional objectives while remaining informed about industry standards and regulatory requirements.

Key Responsibilities

PRIMARY DUTIES AND RESPONSIBILITIES
1. Design, implement, and assess referral processes for rehabilitation patients entering home care.
2. Educate and guide staff to enhance their managerial and operational competencies.
3. Develop and periodically review criteria for patient referrals, collaborating with the Transition Care Nurse (TCN) and Associate Executive Director (AED) to ensure appropriateness.
4. Participate in the orientation of Assistant Directors and other rehabilitation staff in collaboration with the Education department.
5. Evaluate and confirm competency requirements for clinical staff through regular assessments.
6. Direct and supervise rehabilitation staff, serving as both an administrative and clinical resource.
7. Monitor performance of direct reports and conduct evaluations in accordance with HR policies.
8. Initiate progressive disciplinary actions as necessary, ensuring compliance with policies.
9. Communicate program services and policies to the public and healthcare providers, ensuring proper application by staff.
10. Oversee rehabilitation staff assignments, caseloads, service utilization, and productivity metrics.
11. Provide direct supervision to field staff, including conducting on-site visits when required.
12. Maintain and report departmental operational statistics to support strategic planning and budgeting.
13. Conduct documentation management reviews, addressing issues and implementing corrective actions.
14. Ensure timely and accurate documentation by direct reports, including OASIS and care plans.
15. Perform field evaluations of rehabilitation staff and participate in family conferences as needed.
16. Identify staffing needs based on productivity and team census, providing updates as required.
17. Facilitate communication and documentation among patients, agency, medical professionals, and referral sources.
18. Assist in the collection of performance improvement data.
19. Implement performance improvement plans and Quality Assurance Performance Improvement (QAPI) initiatives as necessary.
20. Engage in regular Quality Improvement meetings and clinical record reviews.
21. Supervise both field and office staff responsible for care management and coordination.
22. Ensure compliance with state and federal home health regulations and internal policies.
23. Exercise sound judgment in addressing compliance issues and reporting them as necessary.
24. Ensure participation of direct reports in case conferences and interdisciplinary team meetings.
25. Assign staff to interdisciplinary teams to enhance patient care management.
26. Participate in accreditation visits and external reviews.
27. Monitor key clinical and operational metrics and conduct data analysis for the home care program.
28. Continuously assess work processes and recommend improvements.
29. Stay informed about home health trends and share insights with the team.
30. Collaborate with interdisciplinary team members to develop effective interventions for patient care.
31. Coordinate with schedulers and TCNs for patient referrals and assignments.
32. Ensure the creation and updating of individualized care plans for each patient admitted to the CHHA.
33. Develop and manage special programs and strategic initiatives to enhance rehabilitation services.
34. Conduct regular caseload reviews and chart audits to identify and address deficiencies.
35. Perform additional duties as assigned.
36. Lead case conferences and utilization reviews for admitted cases.
37. Conduct regular reviews of clinician caseloads to implement corrective measures.

Qualifications

REQUIREMENTS
1. Current New York State License and Registration as a Licensed Physical Therapist; and
2. A Bachelor’s Degree from an accredited institution in physical therapy recognized by the Commission on Accreditation in Physical Therapy Education (CAPTE); and
3. A minimum of five (5) years of professional experience in physical therapy, including at least two (2) years in a managerial or supervisory role.
