- Assist clients in establishing and reaching their stability, career, and educational objectives.
- Identify and address barriers to client success proactively.
- Provide holistic service planning and maintain effective communication with agency specialists.
- Stay informed about community resources and share relevant knowledge with your team.
- Utilize a problem-solving approach to foster consistent client progress.
- Support the documentation and evaluative needs of the Padua program.
- Leverage internal and external partnerships to enhance client service plans.
- Work collaboratively with clients and case managers.
- Participate in training and development opportunities.
- Assist clients in accessing necessary resources, including transportation when required.
- Maintain a flexible schedule to accommodate client meetings outside regular hours.
- Monitor program deadlines to ensure compliance with targets.
- Regularly meet with case managers to review client progress and strategize next steps.
- Advocate for clients with community partners to connect them with valuable resources.
- Bachelor's degree in social work is preferred.
- Experience working with low-income populations is essential.
- Familiarity with Salesforce and community resources is advantageous.
- Bilingual in Spanish is required.
- Reliable transportation is necessary for client transport.
