About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager RN to join our team at Heartland Hospice. As a key member of our clinical leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.

Key Responsibilities
  • Administer the clinical aspects of the hospice program, ensuring the provision of quality care to patients.
  • Manage, oversee, and direct the services and performance of assigned team members.
  • Supervise, manage performance, and direct the work of specified team members, assigning admit and evaluation visits, scheduling visits, and ensuring staff are completing visits in a timely manner.
  • Manage the site in the absence of the Executive Director or Administrator.
  • Manage and oversee clinical and administrative functions, including recordkeeping.
  • Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Degree in nursing.
  • Registered Nurse with ASN or BSN from an accredited school of nursing, one-year experience as an RN (Licensed in the state of requested employment).
  • Three years' experience in direct patient care.
  • One year of management experience.
  • Two or more years' experience in hospice or home care required.
  • Must be able to effectively operate nursing assessment tools and office equipment, including computers and tablets.
  • Sensitivity to the feelings of the terminally ill and their families before and after a patient's death.
  • Knowledge of terminally ill patients and their families, along with an understanding of the hospice concept.
