Behavioral Health Specialist

Meridian Health Services is seeking a skilled and compassionate Skill Builder to join our Adult Behavioral Health program in Wayne County, IN. As a key member of our team, you will provide skills and development training to patients and families, helping them achieve mental, emotional, and physical well-being.
Key Responsibilities- Develop and implement individual treatment plans in collaboration with the clinical team.
- Provide individual and group skills training to assist patients and families in developing skills such as self-care, daily life management, interpersonal communication, and problem-solving.
- Conduct case management services as identified in the patient's treatment plan.
- Work collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient.
- Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases.
- Maintain positive working relationships with staff of other agencies and institutions, and act as a liaison between patients and agencies.
- High school diploma or equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework required.
- Associate or bachelor's degree preferred.
- A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
