Position: Community RN - Florida PACE Centers

Miami Jewish Health stands as a leading provider of healthcare services and living solutions for seniors in the Southeastern United States.

We manage the Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, recognized as the largest PACE initiative in Florida, which integrates health insurance, healthcare delivery, and social services tailored for older adults with intricate medical requirements.

Our network includes four adult day health facilities catering to participants across designated counties.

Role Overview:

Under the guidance of the clinic manager RN and the medical director, the Community RN is responsible for delivering nursing care to all individuals enrolled in the program.

Key Responsibilities:

  • Conduct comprehensive nursing assessments and physical examinations for new referrals.
  • Develop suitable care plans and engage in interdisciplinary team discussions.
  • Assess and manage participants experiencing acute health issues.
  • Refer participants to the medical director or specialists as necessary.
  • Actively engage in family meetings and discussions.
  • Contribute to interdisciplinary team meetings, committees, and assigned projects.

Qualifications:

Education:

  • Graduation from an accredited nursing program.

Experience:

  • A minimum of 1 year of RN experience or 3 years of LPN experience.
  • At least one year of experience working with elderly or frail populations.

Licenses/Certifications:

  • Current RN license issued by the Florida State Board of Nursing.
  • Valid Florida Driver's License and proof of insurance.
  • BLS certification is mandatory; ACLS certification is preferred.

Required Skills:

  • Bilingual proficiency in English and Spanish; knowledge of Creole is advantageous.
  • Demonstrated clinical leadership abilities.
  • Strong interpersonal skills for effective collaboration within an interdisciplinary team.

Work Environment:

  • Standard working conditions with adequate lighting and ventilation.
  • Ability to assist with patient transfers and mobility, which may involve lifting and maneuvering patients.

Exposure to Infectious Materials:

  • Routine exposure to blood, bodily fluids, and tissues.

Organizational Expectations:

  • Ensure adherence to patient rights.
  • Exhibit professionalism and accountability in all interactions.
  • Demonstrate a compassionate approach consistent with Miami Jewish EmpathicareSM principles.
  • Maintain confidentiality and security of patient medical information.
  • Participate in departmental meetings and training sessions, adhering to established protocols.
  • Contribute to a safe working environment.
