Position Overview:


This role is situated at MEMORIAL REGIONAL HOSPITAL - (Hollywood, FL) and focuses on facilitating Progression of Care, Discharge Planning, and Interdisciplinary Rounding on the Units to ensure a safe transition for patients to the next level of care.

Weekend and holiday rotations may be required on an as-needed basis.

  • Support the physician and interdisciplinary teams in enhancing patient care, aiming to improve clinical outcomes and patient satisfaction while ensuring effective and timely utilization of inpatient services during transitions of care.

Key Responsibilities:
Assist patients and families in navigating the healthcare system, including post-acute options and insurance plans, to coordinate necessary services.
Collaborate with the interdisciplinary team to continuously assess the patient's current and ongoing needs.
Organize and facilitate care conferences involving patients, families, representatives, and other care team members. Participate in ethics committee meetings as required.
Engage in discharge planning, coordinating patient transfers to other facilities and community resources. Provide discharge education and resource referrals to patients. Address questions and concerns regarding transitions of care.

Work closely with governmental agencies (such as DCF, APS, CMS, and KePro) to ensure safe post-acute care and adherence to regulatory standards.

Education and Certification Requirements:
Graduate of an accredited Registered/Professional Nursing Program (Required)
BLS American Heart E-Card (BLS AHA ECARD) - American Heart Association (AMERICAN HEART)
BLS Certification - American Heart Association (AMERICAN HEART)
Registered Nurse License (RN LICENSE) - State of Florida (FL)

Additional Information:

The complexity of this role necessitates critical thinking skills, effective communication, decisive judgment, and the ability to cultivate positive relationships.

The incumbent must be capable of leading others and taking appropriate actions when necessary.

Must provide education and resources pertinent to the effective progression of care, service utilization, appropriate care levels, and safe patient transitions to both patients/families and the healthcare team.
