Role Summary:
The Registered Nurse Care Coordinator collaborates with physicians and interdisciplinary teams to formulate and implement personalized care plans for assigned patients. This role is essential in monitoring patient progress and addressing any obstacles that may impede optimal care delivery, while also being actively engaged in the discharge planning process.
Key Responsibilities:
1. Engage in discharge planning activities, which include facilitating patient transfers to alternative facilities and coordinating access to community resources.
2. Provide educational resources and discharge instructions to patients and their families.
3. Assess and enhance patient and family satisfaction, addressing inquiries and concerns from patients, family members, and payers regarding care.
4. Make appropriate referrals to Social Workers and identify suitable hospice candidates, communicating effectively with physicians regarding available options.
5. Regularly consult with physicians and multidisciplinary teams to evaluate patient status and the appropriateness of medical interventions, including admission, length of stay, transfer, and discharge.
6. Collect and analyze data to identify quality trends or potential risks affecting patients and staff. Collaborate with multidisciplinary teams and committees to improve quality outcomes.7. Conduct ongoing chart reviews to identify actual or potential issues related to service delivery, patient outcomes, satisfaction, compliance, cost, and reimbursement.

Qualifications:
Educational Background:
Graduate of an accredited Nursing Program (Required)
Current BLS Certification from an accredited provider (e.g., American Heart Association, Red Cross)
Experience:
A minimum of one (1) year of relevant nursing experience is required.
Additional Information:
This position demands critical thinking abilities, effective communication skills, decisive judgment, and the capacity to cultivate positive relationships. The incumbent must demonstrate leadership qualities and take appropriate actions when necessary.
Working Conditions:
This role involves a combination of office work and patient interaction, requiring the ability to perform various physical tasks, including lifting and carrying patients, as well as extensive keyboard use.
