Job Summary

We are seeking a highly skilled and compassionate Mental Health Service Coordinator to join our interdisciplinary team at Pathways Vermont. As a key member of our Assertive Community Treatment (ACT) team, you will provide flexible and community-based services to individuals experiencing mental health challenges, histories of homelessness, and substance use issues.

Key Responsibilities
  • Relationship Building: Engage in relationship building and rapport building with clients, providing services from a client-centered, harm reduction, and strengths-based approach.
  • Referral Services: Assist clients with referrals to community agencies to ensure access to necessary services and resources.
  • Support and Counseling: Provide one-to-one support and counseling on life skills, housing concerns, and health maintenance, including medication support, housekeeping, and financial management.
  • Documentation: Maintain accurate and up-to-date written and computerized records, compile reports, and complete other program documentation.
  • Team Collaboration: Participate in 24/7 on-call services rotation with other team members and attend annual All Staff and Programmatic Retreats.
Requirements
  • Education: Bachelor's Degree required, Master's Degree preferred.
  • Experience: Prior experience working in mental health and/or harm reduction agencies, preferably in community services.
  • Skills: Excellent interpersonal, communication, writing, and computer skills, with the ability to form relationships with marginalized individuals.
  • Qualifications: Valid driver's license, reliable personal vehicle, and current automobile insurance required.

  • Compensation: $20-$21/Hour, with a $3000 Hiring Bonus Available.
