About the Role:

We are seeking a Community Care Worker to join our team at Great Bay Services. As a Community Care Worker, you will provide community-based support to individuals with intellectual and developmental disabilities, promoting their overall community involvement and independence.

Responsibilities:

  • Develop and implement individualized support plans to help our members achieve their goals.
  • Provide transportation for our members in the community, often using your own personal vehicle (mileage is reimbursed).
  • Complete all required paperwork, including daily notes, mileage logs, timecards, progress notes, and incident reports.
  • Attend and participate in all required meetings, including staff meetings and ISA meetings.
  • Support individuals of all abilities, including those with personal care needs and behavioral accommodations.


Requirements:

  • High School Diploma or Equivalent
  • Completion of TB Test
  • Prior experience working with individuals with intellectual and/or developmental disabilities is preferred
  • Certified DSPs are preferred; training will be provided if needed
  • First Aid and CPR certified; training will be provided if needed
  • Valid Driver's License with a legally registered & inspected vehicle
  • Carry mandatory auto insurance of no less than 100K/300K
  • Must pass routine background checks, including criminal, motor vehicle, DHHS, and others required to comply with Medicaid funding
