Hourly Compensation: $17.00-$18.00

Key Responsibilities

  • Facilitate a comprehensive wraparound process for youth and families, providing tailored, coordinated, family-centered care to address the complex needs of youth engaged with multiple child and family services, at risk of out-of-home placement, and facing behavioral and mental health challenges.
  • Coordinate a collaborative process involving the youth, family, service providers, and key members of the family’s support network to develop a care plan that meets the youth's needs.
  • Collaborate with an Independent Evaluator (Licensed Mental Health Professional) to ensure a thorough assessment is conducted and the findings are integrated into the Care Facilitation Team process.
  • In partnership with the family, create a personalized Plan of Care that complies with National Wraparound Initiative standards and treatment planning guidelines.
  • Assist the child/youth and family in accessing strength-based mental health services, social services, educational resources, and other necessary supports to help the family achieve its goals.

Qualifications

  • A Bachelor’s degree from an accredited institution in social services, human services, or behavioral sciences is required. Degrees in other fields will be considered with at least two years of relevant full-time experience in family, children/youth, or community service roles. A Master’s degree is preferred.
  • Prior experience in a relevant family, children/youth, or community service role is mandatory if the degree is not in human services. Certification as a wraparound practitioner must be obtained within two years of employment and maintained thereafter, including annual re-credentialing.
  • Must be willing to travel extensively and drive. Availability for 24/7 on-call crisis intervention for youth is required, along with a flexible schedule that may include evenings and weekends. Ability to transport youth and volunteers is essential.

Eckerd Connects' Wraparound Agencies are integral to the Coordinated System of Care (CSoC) under the state’s behavioral health managed care initiative, serving children and adults in Louisiana with behavioral health, mental health, and substance use disorders. Coordinated Systems of Care are nationally recognized models designed to deliver services in a family-centered manner, ensuring improved outcomes for children, youth, and families. This model encompasses a wide range of flexible and effective services, organized into a coordinated network that integrates care planning and management across various levels and systems. Through this network, we foster meaningful partnerships with families and youth at all levels of service delivery, management, and policy. Services are family-driven and youth-guided, provided in both home and community settings, with a focus on data-driven and outcome-oriented practices. Treatment plans are individualized, strength-based, culturally competent, and integrated across all child-serving systems.
