Compensation: $17.00 - $18.00 per hour

Key Responsibilities:

  • Lead a comprehensive wraparound process for youth and families, ensuring individualized, coordinated, and family-centered care to address the complex needs of youth involved with multiple child and family services.
  • Coordinate a collaborative process involving youth, families, service providers, and key members of the family’s support network to develop a care plan tailored to the youth's needs.
  • Collaborate with a Licensed Mental Health Professional to complete comprehensive assessments and integrate findings into the Care Facilitation Team process.
  • Work with families to create a personalized Plan of Care that meets National Wraparound Initiative standards and complies with treatment planning regulations.
  • Assist families in accessing strength-based mental health services, social services, educational resources, and other necessary supports to achieve their goals.

Qualifications:

  • A Bachelor’s degree in social, human services, or behavioral sciences is required. Degrees in other fields will be considered with at least two years of relevant experience in family, children/youth, or community services. A Master’s degree is preferred.
  • Experience in family, children/youth, or community service roles is mandatory if the degree is outside of human services. A wraparound practitioner certificate must be obtained within two years of employment and maintained thereafter.
  • Willingness to travel extensively and provide 24/7 on-call crisis intervention to youth. Flexibility to work nights and weekends is essential. Ability to transport youth and volunteers is required.
