About the Role

We are seeking a Transition of Care Manager to join our team. In this role, you will work collaboratively with interdisciplinary teams and patients/caregivers to identify and develop safer plans of transition to the next level of care. Your responsibilities will include suggesting observation status, homecare or subacute SNF services as needed, reviewing patient EMRs to ensure documentation supports the plan of care, and obtaining authorizations for acute care transfers.

About You

To be successful in this role, you will need a valid New York State license and current registration to practice as a Registered Professional Nurse issued by the NYSED. A Baccalaureate degree in Nursing or a related health field from an accredited college or university is also required. Certification in Basic Life Support (BLS) through the American Heart Association is a plus.
