Job Description

Summary of Duties:

Transitional care management is critical for patients transitioning out of the hospital setting. Our Transitional Care Management (TCM) program aims to support successful transitions for high-risk patients from inpatient to community settings. As a Transition Care Program Manager, you will play a key role in developing and implementing a comprehensive training and workforce development program for TCM staff.

Key Responsibilities:

  • Design and direct a systemwide training program for TCM staff, ensuring alignment with organizational goals and best practices.
  • Develop orientation programs, training curricula, and operational guides utilizing various pedagogical approaches.
  • Collaborate closely with stakeholders to support the rollout of standardized TCM programs.
  • Foster collaboration with Epic teams to optimize EHR functionalities supporting TCM workflows and documentation.
  • Utilize data analytics to monitor key performance indicators related to training outcomes and staff competency.
  • Regularly visit system hospitals to identify training gaps and provide hands-on training to facility staff.

Minimum Qualifications:

  1. A Baccalaureate Degree in Education, Public Health, Psychology, Personnel Administration, Management, or a related field.
  2. Eight years of experience in planning and manpower development and training programs, medical or public health administration, or other fields related to training and development.

Salary Range: $85,000 - $115,000 per year
