Job Summary

The Care Transition Coordinator RN plays a critical role in ensuring seamless transitions of care for our members. As a key member of our care management team, you will work closely with members, caregivers, and healthcare providers to coordinate care and prevent unnecessary hospitalizations.

About the Role
  • Provide care management support during transitions of care, including assessment, planning, implementation, and evaluation.
  • Collaborate with members, caregivers, and healthcare providers to identify and address gaps in care.
  • Develop and implement individualized plans of care that meet the unique needs of each member.
  • Ensure timely follow-up appointments, medication reconciliation, and education related to chronic diseases and preventive care.
  • Employ critical thinking and judgment when dealing with unplanned issues, such as urgent care or emergency department visits.
Requirements
  • Bachelor's Degree in Nursing required; Master's Degree preferred.
  • Minimum 2 years of care management experience in a healthcare setting or managed care organization.
  • Registered Nurse with current NYS license.
  • Ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports, and prescriptions.
  • Excellent communication and interpersonal skills, with ability to work collaboratively with various team members and stakeholders.
