About the Role

We are seeking a highly motivated and experienced Patient Care Transition Manager to join our team at Option Care Health. In this role, you will play a critical part in ensuring seamless transitions of care for patients, while promoting our services and products to healthcare providers.

Job Responsibilities
  • Relationship Building: Establish and maintain strong relationships with referral sources, including hospitals, medical practices, and other healthcare providers, to increase sales and patient starts.
  • Care Coordination: Collaborate with internal and external partners to ensure quality of service and effective implementation of treatment plans for patients transitioning to home or alternate site care.
  • Sales and Marketing: Utilize sales techniques to educate patients, their families, and facility staff about our services and products, and provide hands-on education to promote timely discharge and therapy independence.
  • Communication: Effectively communicate with agency staff, medical teams, patients, and family members throughout the discharge process to ensure a smooth transition.
Requirements
  • A minimum of 2 years of experience in the healthcare industry, preferably in sales, marketing, or patient care coordination.
  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with various stakeholders.
  • Basic PC skills, including proficiency in Microsoft Office and ability to learn new software applications quickly.
