Job Summary

We are seeking a skilled Care Manager to join our team at Life Care Center of Richland. As a key member of our care team, you will be responsible for coordinating patient care and services, promoting quality care, and ensuring a safe transition from our facility to the next provider or care setting.

Responsibilities
  • Coordinate patient care and services, including case management, utilization review, and discharge planning.
  • Work collaboratively with patients, families, care teams, payers, and external entities to promote quality care and ensure a smooth transition of care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and training to patients, families, and care teams on various aspects of care management.
  • Conduct regular assessments and evaluations to ensure the quality and effectiveness of care services.
  • Collaborate with interdisciplinary teams to develop and implement policies and procedures that promote quality care and patient safety.
Requirements
  • Nursing diploma (associate's or bachelor's degree in nursing) and current licensure/registration in applicable State.
  • One (1) year of clinical experience in post-acute care setting preferred.
  • Prior case management, utilization review, and discharge planning experience preferred.
  • Certified Case Manager (CCM) or Board Certification in Nursing Case Management preferred.
