Job Summary

The Medical Social Worker - Patient Care Specialist is a vital member of the Heartlinks team, responsible for providing comprehensive social work services to patients and their families. Key Responsibilities
  • Assessment and Intervention: Conduct thorough assessments of patients' psychosocial status and develop personalized plans to address their needs.
  • Counseling and Support: Provide counseling and support to patients and their families, addressing stress, coping difficulties, and other related issues.
  • Record Keeping and Reporting: Maintain accurate and detailed records of patient interactions and interventions.
  • Community Resource Referral: Connect patients and families with community resources and services to meet their practical and environmental needs.
  • Liaison and Collaboration: Serve as a liaison between patients, families, and community agencies, ensuring seamless communication and coordination of care.
  • Quality Improvement: Participate in quality assessment and performance improvement initiatives to enhance patient care and services.
Requirements
  • Education: Master of Social Work (MSW) from an accredited institution.
  • Experience: Minimum one year of experience in healthcare, with a preference for hospice experience.
  • Skills: Excellent verbal and written communication, organizational, and interpersonal skills.
  • Certifications: Current CPR certification and valid driver's license.
