Patient Care Coordinator

We are seeking a skilled Clinical Nurse Specialist to join our team at Kadlec. As a key member of our patient care team, you will be responsible for providing high-quality, patient-centered care in a clinical setting.
Key Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care plans.
- Collaborate with healthcare teams to develop and implement patient care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients to identify their physical, emotional, and social needs.
- Develop and implement individualized care plans that address the patient's needs and goals.
- Provide education and support to patients and their families to promote health and wellness.
- Participate in quality improvement initiatives to ensure high-quality patient care.
- Graduation from an accredited nursing program.
- Current Washington Registered Nurse License.
- Current National Provider BLS - American Heart Association certification.
- Recent nursing experience.
- Telephone triage experience.
