Job Description

LHC Group is seeking a talented and compassionate Home Health Physical Therapist to join our team.

Responsibilities
  • Evaluate and assess patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
  • Develop and implement effective treatment plans to improve and enhance patient well-being.
  • Collaborate with healthcare professionals to ensure comprehensive care.
  • Communicate patient progress and response to treatment to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
Requirements
  • Current Physical Therapy licensure in state of practice.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
