Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Philadelphia, Pennsylvania.

Job Summary:

We are looking for a licensed Physical Therapist to provide high-quality, patient-centered care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help our clients achieve their rehabilitation goals.

Responsibilities:
  • Conduct home visits to assess clients' physical abilities and develop personalized care plans
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function
  • Continuously assess and revise care plans to ensure optimal outcomes
  • Collaborate with nursing staff to develop and implement comprehensive care plans
  • Provide education and instruction to clients and their families on rehabilitative care and activities
Requirements:
  • Current PA Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred
