Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Ridgefield Park, New Jersey.
Job Summary:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help our clients achieve their rehabilitation goals.
Responsibilities:- Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
- Provide one-on-one physical therapy sessions to clients in their homes, focusing on exercises, stretches, and other interventions to improve mobility, strength, and function.
- Collaborate with other healthcare professionals, including nurses and occupational therapists, to ensure comprehensive care and coordination.
- Document client progress, update care plans, and communicate with clients, families, and healthcare teams as needed.
- Stay up-to-date with industry developments, best practices, and evidence-based guidelines to provide the highest quality care.
- A current NJ Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
