Your Role:

As a Care Services Coordinator at Arkos Health, you will be instrumental in organizing and facilitating patient care. Your duties will encompass communication with field providers, educating patients about available services, and ensuring effective healthcare delivery. By interacting with patients with empathy and foresight, you will enhance their overall healthcare experience and outcomes.

Primary Responsibilities:

  • Deliver comprehensive telephone support for transitional and post-acute teams
  • Clarify services to patients and secure necessary consents
  • Coordinate schedules for healthcare teams in accordance with contractual obligations
  • Initiate timely communications and documentation for patients
  • Collaborate with various stakeholders to ensure seamless care coordination
  • Accurately organize and maintain patient records

Qualifications and Skills:

  • High school diploma or GED (Some college education or medical certification preferred)
  • Experience in customer service is beneficial
  • Knowledge and experience in healthcare are advantageous
  • Familiarity with medical terminology and patient care
  • Bilingual capabilities are highly regarded
  • Availability on weekends is a plus but not required
