Role Overview
The Health Navigation Specialist plays a crucial role in assisting members to implement sustainable lifestyle modifications that enhance their overall health and wellness. Collaborating with Care Management Registered Nurses, this position works directly with members to develop tailored wellness strategies, reinforce educational initiatives, and provide continuous support to help them achieve their health objectives.
Key Responsibilities:
- Work closely with team members to ensure members have access to vital services.
- Engage with patients and their families to improve participation and tackle challenges throughout their healthcare experience.
- Encourage members to make positive changes in nutrition, physical activity, stress management, and safety practices.
- Manage care referrals for both registered and non-registered participants.
- Evaluate members' needs during telephonic and/or digital interactions and refer them for specialized medical assistance as needed.
- Utilize motivational interviewing techniques to support members in achieving mutually agreed-upon health goals.
- Proactively reach out to eligible members to promote program enrollment.
- Disseminate approved health education materials to participants through various channels.
- Encourage member engagement in preventive care, health screenings, and wellness initiatives offered by the organization or its partners.
- Connect patients and their families with community resources and follow up to ensure their needs are met based on identified social determinants of health.
- Perform tasks under direct supervision, addressing basic issues and escalating more complex matters to senior staff.
- Document referrals and maintain accurate records of communications and actions in the appropriate systems for auditing purposes.
Qualifications:
- An Associate's degree in a healthcare-related field.
- A minimum of 2 years of experience in healthcare (insurance, customer service, or direct patient care) is essential for success in this role.
- Strong interpersonal communication skills are vital as this role involves direct interaction with members.
- Ability to adapt to various situations and demonstrate flexibility.
- A collaborative mindset is essential for working effectively with both members and the Care Coordination team.
- Proficient verbal and written communication skills are necessary for engaging with members and the team.
Equivalent combinations of education, experience, or training may be considered acceptable.
