Your Role:

As a Care Services Coordinator, you will be essential in managing and facilitating patient care. Your duties will encompass communication with field providers, guiding patients through available services, and ensuring the effective delivery of healthcare. Primary Responsibilities:

  • Deliver comprehensive telephonic support for transitional and post-acute care teams
  • Clarify services to patients and secure necessary consents
  • Coordinate schedules for healthcare teams in accordance with contractual obligations
  • Initiate timely communications and documentation for patients
  • Collaborate with various stakeholders to ensure seamless care coordination
  • Accurately organize and maintain patient records

Qualifications and Skills:

  • High school diploma or GED (Some college education or medical certification preferred)
  • Experience in customer service is advantageous
  • Knowledge of healthcare practices and experience is a plus
  • Familiarity with medical terminology and patient care processes
  • Bilingual capabilities are highly valued
  • Availability on weekends is beneficial but not required
