Job Title: Wellness Liaison

Sun River Health is seeking a highly motivated and detail-oriented Wellness Liaison to join our team in Jamaica, Queens, NY. As a key member of our team, you will play a vital role in providing outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services.

Key Responsibilities:
  • Participate in outreach and educational events to promote health services and engage with the community.
  • Act as a liaison between clients, providers, and account management staff to ensure seamless communication and coordination.
  • Provide outbound reminder calls to clients prior to their scheduled appointments and assist with securing appointments for SRH Health Center services.
  • Conduct Health Home pre-eligibility screenings and intakes for eligible clients and achieve monthly intake targets.
  • Assist with client outreach, education of SRH services to the community, and linkage to Health Center and Health Home.
  • Arrange transportation and assist with health insurance applications as needed.
  • Enter data accurately and timely into required databases, including demographic information, insurance information, assessments, and progress notes.
  • Provide necessary documentation and information to Departmental Health Home Manager in each county/borough.
  • Participate in community events, such as health fairs, and conduct on-street marketing of flyers and service information pamphlets.
  • Support SRH's mission, values, strategic goals, and high standards of customer service.
  • Adhere to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
Requirements:
  • Bachelor's degree preferred, High School Diploma/GED required with an additional four years of relevant experience in customer service.
  • Minimum 2-4 years of work-related experience.
