Job SummaryThe Community Liaison Worker is responsible for coordinating psychiatric out-patient appointments, assisting patients with referrals for entitlements and community resources, and collecting data on patient discharges. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and community agencies.
Key Responsibilities- Assist patients with coordinating out-patient appointments and referrals for entitlements and community resources
- Collect data on patient discharges and enter it into the Psychiatric Discharge Management System (PDMS)
- Meet with patients prior to discharge to explain the Koskinas Program and obtain agreement for outreach services
- Track and input housing and treatment outcomes for patients eligible for follow-up
- Call patients and community agencies to determine successful compliance with discharge plans
- Send reminder letters and make supportive interventions to expedite patient follow-up
To be successful in this role, you will need:
- One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services
- Completion of an approved six-month training program in community liaison work, plus six months of full-time experience in a related field
