We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As part of our team, you will work closely with patients and their families to develop personalized care plans that cater to their unique needs.

As a Pediatric Home Health Care Nurse Specialist, your primary responsibility will be to deliver top-notch nursing care to children and adolescents in their homes. You will work collaboratively with our interdisciplinary team to ensure seamless transitions between care settings and coordinate with other healthcare professionals as needed.

This role requires strong clinical skills, excellent communication abilities, and a genuine passion for delivering compassionate care. If you are dedicated to making a positive impact on the lives of vulnerable populations, we encourage you to apply.

Job Requirements:

  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
