Position Overview: The Case Manager for the Transitional Case Management Substance Use Disorder Expansion Program plays a crucial role in delivering comprehensive transition planning and care management services to individuals with substance use disorders. This position is designed for those who are being reintegrated into the community after incarceration.

Key Responsibilities:

  • Provide targeted services to incarcerated individuals aged eighteen and older with documented substance use disorders, both during and after their release.
  • Collaborate with facility staff to identify potential clients who may benefit from transitional case management services.
  • Develop and implement integrated reentry service plans, ensuring clients have access to necessary post-release services.
  • Assist clients in navigating the application process for benefits and entitlements, facilitating connections to aftercare services, housing, vocational training, and peer support networks.
  • Coordinate with licensed treatment facilities and Medication Assisted Treatment providers to ensure clients receive appropriate care upon release.
  • Work in partnership with social services to expedite the activation of Medicaid and other benefits.
  • Maintain accurate and comprehensive client records, ensuring availability for supervisory review.
  • Manage a caseload through consistent client engagement and follow-up.
  • Participate in meetings and ongoing professional development training.
  • Provide transportation or escort services for clients to appointments as needed.

Qualifications:

  • Bachelor's Degree in Social Work, Mental Health Counseling, or Criminal Justice, with a minimum of two years of experience working with substance use and criminal justice populations.
  • Strong knowledge of treatment services and the ability to collaborate effectively with housing, treatment, and mental health systems.
  • Valid driver's license and access to a vehicle are required.
  • Excellent verbal and written communication skills.
  • Proficiency in computer applications, including Microsoft Office and database software, is essential for performing job responsibilities.
