About EAC Network: EAC Network is a non-profit organization dedicated to empowering and assisting individuals in need through a variety of programs across Long Island and NYC.

Key Responsibilities:

  • Deliver services to a targeted demographic of incarcerated individuals aged eighteen and older with documented substance use disorders.
  • Collaborate with the Jail Substance Use Unit staff to identify and accept clients who would benefit from SUD-TCM services.
  • Formulate integrated reentry service plans and ensure these plans are shared with the Jail Substance Use Unit.
  • Support individuals transitioning from incarceration in applying for necessary benefits and connecting them to aftercare services, housing, vocational, and educational resources.
  • Facilitate linkages to OASAS licensed facilities and Medication Assisted Treatment (MAT) providers upon release.
  • Work in conjunction with the Suffolk County Department of Social Services to ensure prompt activation of Medicaid and other benefits.
  • Maintain accurate client records and ensure they are available for review by supervisory staff.
  • Conduct ongoing client contact to manage caseload effectively.
  • Participate in meetings and clinical training sessions.
  • Provide transportation or escort clients to various appointments post-release.

Qualifications:

  • Bachelor's Degree in Social Work, Mental Health Counseling, or Criminal Justice with a minimum of two years of experience in working with substance use and criminal justice populations, or hold a CASAC certification.
  • Familiarity with available treatment services and the ability to collaborate effectively with housing, treatment, and mental health systems.
  • Possession of a valid New York State Driver's License and access to an automobile.
  • Strong communication skills, both verbal and written.
  • Proficiency in computer applications, including spreadsheets and Microsoft Office, necessary for job responsibilities.
