POSITION SUMMARY

The Clinical Care Coordinator is responsible for overseeing and ensuring the prompt and precise execution of the RAI and Care Management processes from the point of admission through to discharge, adhering to the policies and procedures of the organization, as well as relevant state and federal regulations. This role may also involve coordinating the operations and educational initiatives of the information systems within the clinical department.

QUALIFICATIONS
  • RAC Certification required
  • Registered Nurse (RN) license mandatory
  • Must maintain good standing with the Board of Nursing
  • Preferred knowledge of Medicare processes
  • Current state nursing license required
  • Continuing Education Units (CEUs) as mandated by state regulations
KEY RESPONSIBILITIES

The following duties are subject to the specific policies and practices of the facility, which may necessitate additional responsibilities:
  • Collaborates with the Interdisciplinary Team to evaluate resident needs
  • Receives guidance from the Regional Clinical Reimbursement Consultant
  • Facilitates daily care management meetings, reviewing resident care and establishing the Assessment Reference date
  • Ensures timely and accurate resident assessments in compliance with state and federal guidelines
  • Assigns responsibilities for completing assessment sections to the interdisciplinary team
  • Accurately codes the MOS using information from medical record reviews, observations, and interviews with staff, residents, and families
  • Maintains a tracking system for MOS schedules, including time frames and due dates
  • Ensures precise data entry of resident information into relevant computerized MOS programs
  • Coordinates care plan meetings with the interdisciplinary team, residents, and families
  • Obtains, reviews, and maintains all state and federal reports, making necessary corrections promptly
  • Facilitates educational sessions regarding the RAI/Care Management process for the facility
  • Oversees the electronic submission of required documentation to state databases and other entities in accordance with company policy and regulations
  • Completes necessary forms and documentation in line with company policy and state/federal regulations
  • Participates in Interdisciplinary Team meetings to gather information, communicate updates, and maintain records
  • Contacts RCRC for Medicare compliance inquiries
  • Continuously enhances knowledge related to data entry and computer technology
  • Engages in quality assurance activities
  • Submits required documentation electronically to state databases and other entities as per company policy
  • Performs additional duties as assigned
WORK ENVIRONMENT AND PHYSICAL DEMANDS

This position is primarily based in a standard office environment. The employee may be required to sit, bend, stoop, use a keyboard, and engage in verbal communication. Occasionally, the employee may need to lift medium to heavy objects, including assisting with resident mobility. There may be exposure to blood or other bodily fluids, fumes, airborne particles, and hazardous chemicals.
