Responsibilities:

  • Provide individualized services to people with disabilities and their families, including respite, teaching, and skill development.
  • Participate as an active member of the person's team, providing ideas, input, and advocacy for the person related to their Staff Action Plan.
  • Follow objectives as described in the person's Staff Action Plan and as discussed with supervisor.
  • Attend training to improve services provided.
  • Complete job requirements, including training and PPD, within appropriate time frames.
  • Report any potential incidents to the designated Family Support Coordinator/Supervisor immediately.
  • Accurately enter time and services into Racker's electronic health record, eVero.

Qualifications:

  • High school diploma or GED.
  • Ability to meet Racker's approved driver criteria.
