We are seeking an experienced Clinical Care Coordinator to join our team. As a Clinical Care Coordinator, you will play a vital role in ensuring the highest quality of care for our residents.

Job Summary:
The Clinical Care Coordinator will be responsible for maintaining and following a schedule of due dates for all Minimum Data Sets (MDS). They will coordinate the completion of MDS by all disciplines, review data for accuracy, and meet with staff as needed to ensure accuracy. Additionally, they will complete MDS and input it into the computer, verify assessments have been transmitted and approved by the state in a timely manner, correct rejected records, and prepare them for re-submission. The Clinical Care Coordinator will also formulate Resident Care Plans (RCP) along with the interdisciplinary care plan team, schedule and conduct RCP meetings, communicate plans to staff members, and work with departments to ensure understanding and compliance with RCP.

Responsibilities:
- Maintain and follow a schedule of due dates for all MDS
- Coordinate the completion of MDS by all disciplines
- Review MDS data for accuracy and meet with staff as needed to ensure accuracy
- Complete MDS and input it into the computer
- Verify assessments have been transmitted and approved by the state in a timely manner
- Correct rejected records and prepare them for re-submission
- Formulate Resident Care Plans (RCP) along with the interdisciplinary care plan team
- Schedule and conduct RCP meetings
- Communicate plans to staff members
- Work with departments to ensure understanding and compliance with RCP

Requirements:
- Registered Nurse (RN) license from the North Carolina Board of Nursing
- Ability to read, know, and follow personnel, department, and facility policies and procedures
- Experience with MDS-RAP and Care Planning functions
- Prefer experience with RUG-IV and MDS 3.0

Estimated Salary: $65,000 - $85,000 per year
