MDS Coordinator (LPN or RN) at Brickyard Healthcare

The MDS Coordinator plays a critical role in ensuring patients receive necessary care while optimizing resource allocation. This position entails completing the Resident Assessment Instrument according to regulations, serving as an in-house case manager, and coordinating care with various stakeholders.

Key Responsibilities:

  • Responsible for the completion of the Resident Assessment Instrument per federal and state regulations and company policy.
  • Acknowledges all aspects of resident care and coordinates services with physicians, families, third-party payers, and facility staff.

About $85,000 annual salary is expected, considering industry standards and local cost of living.

We rely on our MDS Coordinator to ensure our patients receive the necessary care and that we have the required resources to provide it.

Salary Details:

$80,000 - $90,000 annual salary range based on qualifications and experience.

Additional incentives include shift pick up and more available opportunities.

Requirements:

To be successful in this role, you will need:

  • Licensed Practical Nurse or Registered Nurse with a current, active license in the state of practice
  • Must hold and maintain a current CPR certification
  • Minimum two years of clinical experience in a healthcare setting
  • Minimum of one year of experience in a long-term care setting
  • Prior experience as an MDS coordinator with demonstrated expertise
  • Ability to maintain regular attendance
