Position Overview
NRA is seeking a qualified Practical Nursing Specialist to manage case oversight for our growing services.
In this engaging role, you will be instrumental in the intake and evaluation processes for new referrals, ensuring the formulation and implementation of comprehensive care strategies.
This position offers flexibility and independence in applying nursing assessment, care planning, and case management expertise to provide optimal services in home environments.
Collaboration within a respectful and compassionate team is essential. This role does not involve performing skilled nursing tasks.
Qualifications for this position include a valid MA LPN license, computer proficiency, travel and organizational skills, flexibility, excellent communication skills, as well as training, referral, and planning experience.

Key Responsibilities
- Conduct Intake and Assessments
- Perform comprehensive intake evaluations for new referrals and follow-up assessments as necessary.
- Execute nursing assessments and meet Prior Approval criteria as per members' insurance policies.
- Provide Supervision and Training
- Deliver guidance, training, and evaluations to caregivers to ensure high-quality care delivery.
- Foster a collaborative and supportive atmosphere within the caregiving team.
- Develop Individualized Care Plans
- Create and maintain detailed Individualized Care Plans tailored to each member's unique needs.
- Ensure plans are routinely reviewed and updated to reflect any changes in health or circumstances.
- Conduct On-Site Visits
- Visit each member onsite as required or more frequently if necessary.
- Monitor the health status and general needs of all members and caregivers, making referrals when appropriate.
- Participate in Meetings
- Engage in various meetings adequately prepared as needed.
- Collaborate with team members, caregivers, families, and other relevant parties.
- Manage Reporting and Documentation
- Report any changes in members' health status to their physician, director, and other relevant parties.
- Complete nursing progress notes during each visit and fulfill other documentation requirements as per service type.
- Plan Discharges
- Develop and implement appropriate discharges or care transitions in collaboration with the healthcare team.
- Provide Education and Training
- Offer ongoing educational information and training to all members and caregivers on health and aging.
- Deliver member-specific training and address current health/public health conditions.
- Participate in the On-Call System
- Be part of the on-call system as scheduled, providing support as necessary outside of regular business hours.
- Weekend Availability
- Be available to work weekends multiple times throughout the year to meet the needs of the individuals supported.
- Driving Requirement
- Capable of driving a motor vehicle to the homes of those being served.
- Teamwork and Essential Responsibilities
- Collaborate effectively within the agency, office, team, and with all relevant stakeholders.
- Undertake any and all duties deemed essential to support the agency in achieving its mission.
Qualifications
- Current MA Licensed Practical Nurse (LPN) License.
- Proficiency in computers and organizational planning.
- Two years of recent experience with elders, individuals diagnosed with ID/D, ABI, or chronic conditions impacting their ability to live safely in the community.
- Effective interpersonal and communication skills.
- Ability to work harmoniously in a team environment.
- Community engagement involving the Department of Developmental Service, Adult Foster Care Services, and HCBS waivers is advantageous.
- Background Check Mandated.
- Valid driver's license with an excellent RMV record (subject to RMV background check).
