About the Role:

As a Care Coach Connect RN Case Manager, you will play a vital role in coordinating care and providing ongoing nursing assessments for patients and their families. You will work closely with interdisciplinary teams to develop and implement individualized plans of care, ensuring that patients receive the best possible outcomes. Your expertise and compassion will make a significant difference in the lives of our patients and their loved ones.

Responsibilities:

  • Conduct medical home visits and urgent/acute visits to assess patient needs and develop care plans
  • Provide ongoing assessments of patient physical, functional, psychosocial, and environmental needs
  • Implement individualized plans of care and recommend revisions as necessary
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Document patient progress and outcomes in the electronic health record
  • Participate in educational offerings to stay up-to-date on chronic medical conditions and licensing requirements

Requirements:

  • Registered Nurse with a current license to practice in the state of employment
  • Current BLS certification or must obtain certification within 30 days of employment
  • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting
  • Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook
  • Excellent nursing skills and ability to interact productively with individuals and multidisciplinary teams

Preferred Qualifications:

  • Hospice and Palliative Care Nursing certification
  • 2+ years of hospice experience
  • Experience with home visits, palliative and hospice medicine, and advanced care planning
