Job Description:

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at the Texas Department of Aging & Disability Services. As a Nurse III, you will play a critical role in providing high-quality care to individuals with intellectual and developmental disabilities who are medically fragile or have behavioral problems.

Key Responsibilities:
  • Conduct comprehensive physical assessments and develop individualized care plans to ensure optimal health outcomes.
  • Collaborate with the interdisciplinary team to coordinate care and services, including medical appointments and treatment.
  • Provide education and support to individuals, families, and staff members on health-related topics and best practices.
  • Participate in quality assurance reviews and contribute to the development of nursing policies and procedures.
  • Maintain accurate and up-to-date records, including individual plans of care and comprehensive quarterly and annual assessments.
Requirements:
  • Current RN licensure in the state of Texas.
  • Knowledge of nursing techniques, standards, and procedures, as well as health care laws and regulations.
  • Ability to work independently and productively with all levels of staff, including direct care staff, LVNs, and other healthcare professionals.
  • Excellent verbal and written communication skills, with the ability to present ideas clearly and concisely in both written and oral communications.
  • Ability to maintain current CPR certification and perform cardiopulmonary resuscitation as needed.
