Exciting Opportunity for a Nurse Practitioner or Physician Assistant in Geriatric Care
At Provider Solutions & Development, we are dedicated to delivering exceptional healthcare services to elderly individuals through our innovative programs. We are currently seeking a full-time Nurse Practitioner or Physician Assistant with a minimum of 1 year of experience in geriatric care. In this role, you will manage a select group of patients at your primary location while also providing support to various other facilities in the area.
- Full-time position at 1.0 FTE
- Monday - Friday schedule with daytime hours
- Work in an outpatient primary care setting, conduct home visits, and engage with community-based services
- Collaborate closely with a multidisciplinary team including physicians, nurse care managers, social workers, pharmacists, and behaviorists
- Preference for candidates with 2 years of post-graduate clinical experience in geriatric or hospice care, with a minimum of 1 year required
- Ideal candidates will possess a background in palliative care
