Exciting Career Opportunity as a Nurse Practitioner or Physician Assistant in Geriatric Care

At Provider Solutions & Development, we are dedicated to delivering exceptional healthcare services to the elderly population. We are seeking a full-time Nurse Practitioner or Physician Assistant with a minimum of one year of experience in geriatric care. In this role, you will manage a small patient panel at your primary location while also providing support to various other facilities within the network.

  • Full-time employment at 1.0 FTE
  • Standard Monday to Friday work schedule with daytime hours
  • Engage in outpatient primary care, conduct home visits, and attend community-based settings
  • Collaborate with a multidisciplinary team including physicians, nurse care managers, social workers, pharmacists, and behavioral health specialists
  • Preference for candidates with two years of post-graduate clinical experience in geriatric or hospice care, with a minimum of one year required
  • Ideal candidates will possess experience in palliative care
