The Primary Care Manager RN is responsible for delivering comprehensive care management, coordination, and skilled nursing services to at-risk elderly patients facing complex medical and psychosocial challenges. Utilizing population-based disease management strategies and clinical protocols is essential to this role. This position is integral to the patient-centered PACE interdisciplinary team (IDT), ensuring a systematic approach to health service coordination throughout the care continuum, aimed at enhancing patient experiences, managing healthcare costs, and improving overall quality of care.
As part of the PACE program, we focus on keeping older adults healthy and active within their communities through various services, including clinics and home visits. This role empowers elders to maintain their independence and quality of life.
Essential Qualifications:
- Oregon Registered Nurse License upon hire
- National Provider BLS certification from the American Heart Association within 30 days of hire
- A minimum of 2 years of RN experience in PACE, home health, skilled nursing, community health, hospice, or related fields
Preferred Qualifications:
- Bachelor's Degree in Nursing
- Current recognition by a national professional organization in care management, geriatrics, or hospice upon hire
