Exciting Opportunity for a Nurse Practitioner or Physician Assistant in Elder Care
At Provider Solutions & Development, we are seeking a dedicated Nurse Practitioner or Physician Assistant to join our esteemed team focused on delivering exceptional healthcare services to elderly individuals. This full-time position requires a minimum of 1 year of experience in geriatric care, where you will manage a select group of patients at your primary practice while also providing support to various locations.
- Full-time employment at 1.0 FTE
- Standard Monday to Friday work schedule with daytime hours
- Engage in outpatient primary care, conduct home visits, and serve in community-based settings
- Collaborate with a multidisciplinary team including physicians, nurse care managers, social workers, pharmacists, and behaviorists
- Preferred candidates will have 2 years of post-graduate clinical experience with geriatric or hospice patients; 1 year is mandatory
- Background in palliative care is highly desirable
- Attractive salary package, performance bonuses, relocation assistance, and continuing medical education (CME) funds/hours available
