Renal Care Specialist

We are seeking a skilled and compassionate Renal Care Specialist to join our team at Henry Ford Health System. As a key member of our dialysis unit, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Responsibilities- Conduct thorough assessments of patients to identify their unique needs and develop individualized care plans.
- Implement and evaluate the effectiveness of care provided to patients, ensuring that their physical and psychosocial needs are met.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between departments.
- Provide education and support to patients and their families to promote self-care and independence.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduate of an accredited nursing program.
- Current RN licensure in the State of Michigan.
- At least 12 months of experience in nursing care, with 3 months of experience providing care to maintenance hemodialysis patients.
