JOB TITLE: Chaplain

REPORTS TO: Patient Care Manager/Executive Director

POSITION SUMMARY: The Chaplain plays a crucial role in helping patients and their families navigate spiritual concerns associated with end-of-life experiences, such as meaning, relationships, hopes, and fears. The Chaplain offers solace and support by honoring each individual's spiritual beliefs and/or faith traditions.

KEY RESPONSIBILITIES:

  1. Conduct thorough spiritual evaluations of patient and family units to identify needs and dynamics, documenting findings in accordance with hospice standards.
  2. Collaborate as a member of the interdisciplinary team (IDT) to develop a comprehensive care plan for patients and families in consultation with other team members.
  3. Observe and assess psychosocial, spiritual, physical, and financial conditions affecting the patient and family, sharing insights during team meetings.
  4. Evaluate the spiritual needs of patients and family members as appropriate.
  5. Provide individualized and family spiritual support at regular intervals aligned with the patient's care plan.
  6. Coordinate spiritual care efforts with the hospice team and the patient's designated clergy.
  7. Maintain accurate and current documentation of all family interactions and interventions, ensuring up-to-date patient records and care plans.
  8. Contribute to effective communication, fostering a positive work environment and teamwork.
  9. Commit to ongoing professional development and skill enhancement, fulfilling assigned responsibilities, including knowledge of organizational policies and information shared during agency meetings and training sessions.
  10. Participate in agency-wide initiatives and activities as assigned.
  11. Demonstrate a commitment to enhancing service quality and engage in the Quality Assessment Performance Improvement Program (QAPI) as requested.
  12. Integrate QAPI principles into the spiritual care components.
  13. Exhibit flexibility and accept additional duties as needed.
  14. Attend mandatory training and educational sessions.
  15. Uphold patient confidentiality in accordance with applicable regulations.

JOB EXPECTATIONS:

  1. Maintain confidentiality of patient and family information, adhering to HIPAA regulations.
  2. Exhibit self-management skills, including time management, communication, and tracking results while managing multiple priorities.
  3. Demonstrate the ability to work effectively with diverse organizations and individuals.
  4. Possess strong organizational, prioritization, assessment, and communication skills.

QUALIFICATIONS:

  1. Dedication to meeting the needs of terminally ill individuals.
  2. Ordination or equivalent spiritual/pastoral training is required.
  3. Experience in hospice, hospital chaplaincy, or working with terminally ill patients and their families is preferred.
  4. Ability to assess spiritual needs and initiate appropriate interventions.
  5. Knowledge and respect for a wide range of spiritual and religious traditions.
  6. Ability to collaborate effectively as part of an interdisciplinary team.
  7. Strong written, verbal, and interpersonal communication skills.
  8. Proficient in email, word processing, internet navigation, and keyboarding, with the capacity to learn medical software for accurate and timely documentation.
  9. Possession of a valid Driver's License and access to a reliable, insured vehicle.

OSHA CATEGORY:

Category II: Tasks that involve no exposure to blood, body fluids, or tissues, but employment may require performing unplanned Category I tasks.
