ROLE SUMMARY
The Hospice Physician plays a crucial role in offering comprehensive medical guidance and clinical knowledge to the organization, ensuring high-quality end-of-life care for patients. This role promotes a collaborative environment among all medical and nursing personnel involved in hospice care and fosters the integration of primary and palliative medicine by establishing connections with referring primary care physicians and specialists. As a vital member of the interdisciplinary team, the Hospice Physician participates in scheduled IDT meetings, providing medical oversight, directing the clinical team's efforts, and ensuring adherence to best practice standards in hospice care.

This position also serves as a backup for the Hospice Medical Director, providing on-call and vacation coverage as necessary.

REPORTING TO: Executive Director and Hospice Medical Director

KEY RESPONSIBILITIES:

  • Provide clinical leadership and care under the guidance of the Hospice Medical Director, with supervision from the Executive Director.
  • Certify patients' medical eligibility for hospice services by utilizing available diagnostic and prognostic indicators, current medical findings, and treatment orders.
  • Recertify patients as needed, based on face-to-face evaluations required for the continuation of the Medicare Hospice Benefit.
  • Participate in IDT meetings.
  • Collaborate with the patient's attending physician and the hospice IDT to review and update the patient's individualized care plan, ensuring that all medical needs are addressed.
  • Offer consultation, support, and education to the hospice IDT, attending physicians, and colleagues regarding medical eligibility, symptom management, and other relevant issues.
  • Ensure 24/7 physician support is available to hospice RNs, either directly or through collaboration with other qualified providers.
  • Document care provided in the patient's clinical record, including evidence of disease progression. Review medications and treatments to confirm their medical necessity and cost-effectiveness.
  • Lead clinical initiatives in the development and review of care protocols, maintaining current knowledge of research and trends in hospice care and symptom management.
  • Administrative and Operational Duties
  • Ensure continuity of hospice medical services.
  • Guarantee that patients receive appropriate measures for symptom management and participate in the development of their hospice care plans.
  • Actively engage in the organization-wide Quality Assessment and Performance Improvement (QAPI) Plan as directed.
  • Contribute to the development and updating of patient care policies and emergency procedures, and assist in resolving clinical and ethical concerns.

QUALIFICATIONS

Required:

  • Licensed physician (MD or DO) in the State of Oregon
  • Drug Enforcement Administration (DEA) license
  • Valid driver's license and automobile liability coverage, along with several years of progressive medical leadership experience
  • Must reside within a reasonable distance for in-person visits and meetings

Preferred:

  • Five years of clinical experience in geriatrics and/or hospice and palliative medicine
  • Three years of experience providing primary care for hospice patients
  • Experience with medical home visits
  • Familiarity with medical ethics and quality improvement
  • Board Certification in hospice and palliative care is strongly preferred.
