Position Overview:
The Hospice Physician at Hospice of Redmond plays a crucial role in enhancing the quality of life for patients and their families by delivering exceptional hospice, Transitions, Palliative Care, and Bereavement services within the Central Oregon community.
Key Responsibilities:
The Hospice Physician is tasked with providing comprehensive medical leadership and clinical expertise to ensure the delivery of high-quality end-of-life care. This role fosters a collaborative environment among all medical and nursing personnel involved in hospice patient care, promoting the integration of primary and palliative medicine through effective relationships with referring primary care providers and specialists.
This position is vital to the interdisciplinary team, participating in scheduled IDT meetings to offer medical oversight, guide clinical team efforts, and ensure adherence to best practice standards for hospice care.
Additional Duties:
• Serve as a backup to the Hospice Medical Director, providing on-call and vacation coverage as necessary.
• Certify patient eligibility for hospice services based on diagnostic indicators, medical findings, and treatment orders.
• Recertify patients as needed for the continuation of the Medicare Hospice Benefit.
• Collaborate with the attending physician and hospice IDT to review and update individual care plans, ensuring that all medical needs are addressed.
• Provide consultation and education to the hospice IDT and attending physicians regarding medical eligibility and symptom management.
• Ensure 24/7 physician support for hospice RNs, either directly or through collaboration with other qualified colleagues.
• Document care provided in patient records, including evidence of disease progression and review of medications for necessity and cost-effectiveness.
Administrative Responsibilities:
• Ensure continuity of hospice medical services and appropriate symptom management for patients.
• Participate in the organization-wide Quality Assessment and Performance Improvement (QAPI) Plan.
• Contribute to the development and revision of patient care policies and emergency procedures, and assist in resolving clinical and ethical concerns.
Qualifications:
• Required: Licensed physician (MD or DO) in the State of Oregon, DEA license, valid driver's license, and several years of progressive medical leadership experience.
• Preferred: 5 years of clinical experience in geriatrics and/or hospice and palliative medicine, 3 years of experience in primary care for hospice patients, experience with medical home visits, and board certification in hospice and palliative care.
