**Job Summary

We are seeking a highly motivated Licensed Practical Nurse (LPN) to join our team at Foundation Health Partners. As an LPN, you will play a critical role in delivering high-quality patient care and supporting the healthcare team.

The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a dynamic and ever-changing environment.

**Key Responsibilities:

  • Contribute to the development and implementation of individualized care plans
  • Collaborate with the healthcare team to ensure seamless patient care
  • Provide education and support to patients and families
  • Maintain accurate and up-to-date records

**Requirements:

  • Current LPN license in good standing
  • BLS certification
  • 1 year of experience in an acute care setting

**Salary and Benefits:**$65,000 - $85,000 per year, Comprehensive benefits package, including medical, dental, and vision insurance, 401(k) plan, and paid time off.
